Abstract: The paper refers to an event occurred inside an electric substation, happened at the restoration of the electrical supply of an oil-filled transformer subsequent to an intervention of periodic maintenance, and consisting of the outburst of the oil tank with leaking of the diathermic oil both as a high temperature vapor and as liquid on fire. The development of the case study we focus on shows the event chain rebuilt with high reliability and proposes the adoption of preventive measures for the safety of the workers.
